In order to ensure the accuracy and confidentiality of student records, The College requires proof of identification and proper supporting documents in order to change or update demographic information. Students wishing to change their name, address, or personal email address must use the Demographic Change Form and submit where required supporting documentation as listed on page two of the form. All PDF forms must be sent from your Owl email address. Once you have completed your forms accompanied by supporting documentation please send the documents as an attachment to studentservices@ucc.edu
